Assessor Information Notice

The company director responsible for completion and return of these forms sustained a second stroke shortly before this notice requesting information arrived.

This rendered the company director further disabled than was previously the case (spinal surgery ending an earlier career as a police officer).

Consequently, the director would wish to apologise for the late filing of this form.

In addition, the company director is approximately 80% housebound. Given the disabilities, proof of which are contained in this personal website, the company director respectfully requests that today, 20th November 2025, “reasonable adjustmrnts” are made and this internet version of the completed form is accepted by the Assessor, in order that the current £200 late filing penalty does not escalate to over £1,000 tomorrow, Friday 21st November 2025. P.S. Medical advice from the director’s GP and Consultant Neurologist includes regular blood pressure readings throughout each day to prevent further hypertensive (elevated blood pressure to a lethal level). As a consequence, the director is no longer able to send nor receive emails.

Hence this unusual format in returning the requisite information…

Please Note: The company director is struggling with this, and also has to have his grammar and syntax etc., checked by a colleague due to post stroke fatigue and dyslexia.

Consequently, before the full 10 pages are complete and added here this aftetnoon, the director is checking (by phone call to the SAA) and with the very helpful staff at the Assessor’s office whether this format is legible and acceptable in terms of Equality Act 2010 “reasonable adjustments.”
